Community Health Assessement

Laurel County Health Department began partnering with Saint Joseph London in May 2011 to begin the process of completing a community health assessment.

In October 2011, the Laurel County Community Health Needs Assessment Coalition was established. The coalition began working on the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) at that time. The coalition decided to utilize the Mobilizing Action Through Planning and Partnership’s model to complete the process every 3 years.

Becoming Laurel County Health in Motion

In April 2015, the coalition officially became the Laurel County Health In Motion Coalition and has a membership of over 20 community agencies. Laurel County Health Department is a proud and active member of the coalition.
